All new starter information and forms that require completion before you join the Trust will be sent to you via Trac.

Please complete all new starter forms and return by responding to the offer letter email from Trac as soon as possible. Failure to do so may mean your start date will be delayed and your pay will be affected.


All documents need to be sent to Medical HR as soon as possible, they need to be returned via your Trac account. Failure to send the documents and complete all pre-employment checks may mean you will not get paid on pay day. For further instructions, please refer to your original email.

Please ensure you complete the Occupational Health information received via a separate email via Cohort.

If you would like to become a Member of our Trust please complete the form available here

You can become a staff member if you are on a permanent contract, or have a fixed-term contract of at least 12 months by Guy’s and St Thomas’

Payslips are only available electronically via Employee Self Service. You will have access to ESR the day before pay day once your assignment has been created. The information leaflet on how to access Employee Self Service can be downloaded here
