Once you have received your offer letter, contract and generic work schedule it is important to complete the New Starter Forms found below:

New Starter Form

These include a New Starter personal details form, New Starter P46 form, New Starter Pensions Questionnaire and WTR Opt Out Form.


All documents need to be sent to Medical HR as soon as possible. Failure to send the documents and complete all pre-employment checks may mean you will not get paid on pay day. For further instructions, please refer to your original email.

Please ensure you complete the Occupational Health information received via a separate email via Cohort.

If you would like to become a Member of our Trust please complete the form available here

You can become a staff member if you are on a permanent contract, or have a fixed-term contract of at least 12 months by Guy’s and St Thomas’

Payslips are only available electronically via Employee Self Service. You will have access to ESR the day before pay day once your assignment has been created. The information leaflet on how to access Employee Self Service can be downloaded here
