Pre-Employment Checklist

To ensure you are paid correctly and promptly, please take a few minutes to read through the below checklist to ensure you have completed all of your pre-employment actions:

Medical HR – New Starter Documentation and ID Checks

We require pre-employment check documents to be sent to us prior to your start date. You should return these documents via email by response to your conditional offer letter.

Please return scanned colour copies of your documents as listed below:

  • Current Passport
  • Visa for right to work in the UK (if applicable) – In order to prove your right to work in the UK, we require your visa sharecode. Once you have received your visa, you can retrieve your sharecode here: www.gov.uk/prove-right-to-work
  • Enhanced DBS certificate dated within the last 3 years – Please let us know if you don’t have the Enhanced DBS certificate dated within the last 3 years, and we can initiate one for you.  If you are signed up to the online update service, please provide us with the certificate number, date of issue and confirmation that you are in agreement that we can access you DBS via the online service.
  • Two proofs of current address – Options:  driving licence OR utility bill, bank statement, credit card statement, mobile phone bill (dated within three months) OR tenancy agreement, mortgage statement, HMRC letter (dated within one year)
  • Copy of the most recent NHS Payslip (this is to determine if you are eligible for pay protection under schedule 14)
  • New starter paperwork – Please also complete and return all the New Starter Forms attached to this email, returning via your unconditional offer email.

Important – If you do not have a National Insurance Number, please apply for one here: https://www.gov.uk/apply-national-insurance-number

Failure to have the required documents prior to your start date, and failure to attend an ID check will result in your start date being delayed and may impact your pay.
